유방암 환자에서 항암 치료 중 생긴 체중 변화와 재발과의 상관 관계

ABSTRACT

Purpose:

Weight change during chemotherapy is reported to be associated with a worse prognosis in breast cancer. Our study was aimed to evaluate the relation of weight change during AC (anthracycline+cyclophosphamide) +taxanes chemotherapy and recurrence.

Methods:

Patients included 89 women diagnosed with breast cancer who have been treated by AC+taxenes chemotherapy regimen. The weight variation between prechemotherapy and postchemotherapy was calculated ([weight at postchemotherapy-weight at prechemotherapy]/weight at prechemotherapy×100) and categorized into either weight change (≥5%) or stable (<5%). And then, we evaluated the relation of weight change and recurrence through the radiologic image tests (positron emission tomography-computed tomography, breast sonography, mammography, bone scan, magnetic resonance imaging, abdomino-pelvic computed tomography) each group.

Results:

During chemotherapy, 37of total 89 patients (41.6%) presented notable weight change and 52 patients (58.4%) were not weight change. Median follow-up period was 45.1 months, 17 of total 89 patients (19.1%) presented recurrence on the radiologic image tests. Eight of 17 recurrence patients were presented notable weight change, 9 patients were not weight change. In univariate analyses, weight change was not associated with recurrence.

Conclusion:

In our study, weight change during AC+taxanes chemotherapy in breast cancer was not associated with recurrence. Many studies that relation of weight change and recurrence were presented various result. So, we need more clinical studies many patients included and well designed.

서 론

유방암으로 진단받은 환자들을 대상으로 종양의 임상학적 특성에 대한 많은 연구가 시행되면서 유방암의 예후에 영향을 주는 인자들이 밝혀지고 있다[1]. 나이, 종양의 크기, 액와 림프절 전이 유무, 원격 전이 유무, 혈관 침윤 유무, 림프관 침윤 유무, 종양의 핵 등급과 조직학적 등급 등 다양한 인자들이 유방암의 예후 인자로 보고되고 있다[2-5]. 다양한 연구에서 유방암 진단 당시에 과다 체중이나 비만인 경우는 높은 재발률 및 사망률과 관련해 예후가 좋지 않다는 보고들이 있다[6]. 항암치료 중 발생하는 체중 변화와 예후의 상관 관계를 분석한 다른 연구들에서 보면 몇몇 연구에서는 체중 증가가 전체 생존율 감소 및 재발률의 증가와 관련이 있다는 보고가 있으나 다른 연구에서는 연관성이 없다는 보고도 있다[7-11].
이에 본 연구는 유방암으로 수술 후 항암 치료를 받은 유방암 환자에서 발생한 체중 변화와 재발과의 연관성을 분석하여 항암 치료 중 발생한 체중 변화가 유방암의 예후 인자로서의 의미가 있는지를 알아보고자 하였다.

방 법

유방암으로 수술(유방 보존술, 변형 근치 유방 절제술) 후 전신 항암요법을 받은 환자 중 진단 당시 원격 전이가 있는 환자들을 제외한 stage II, III에 해당하는 환자에서 anthracycline+cyclophosphamide(AC)+taxenes 항암 요법 치료를 받았고 체중 분석을 하기 위한 의무기록이 정확하게 입력되어 있는 89명의 환자를 대상으로 하였다. 환자들은 AC항암 요법 치료를 3주 간격으로 4회 시행 후 taxanes계열 항암제를 3주 간격으로 4회 시행하여 총 8회의 항암 치료를 받았으며, 항암 치료 도중 부작용이나 환자의 치료 거부 등으로 치료를 완료하지 못한 환자들은 조사 대상에서 제외하였다.
이 환자들이 치료를 시작할 때의 체중과 종료할 때의 체중에 대하여 조사하였고, 그 외에 나이, 종양의 크기, 액와 림프절 전이, 호르몬 수용체 발현 여부, HER2/neu 발현 여부, body mass index (BMI)를 조사하였다. 환자들은 항암 치료 종료 후 주기적인 영상의학 검사 (positron emission tomography-computed tomography, breast sonography, mammography, bone scan, magnetic resonance imaging, abdomino-pelvic computed tomography)를 시행하였고, 검사상 나타난 결과의 영상의학과 전문의 판독을 통해 재발 및 전이 여부를 조사하였다.
체중변화는 매 항암 치료 시마다 전자 체중계를 통해 측정된 의무기록상 체중을 조사하여 항암치료 시작할 때와 종료할 때의 차이로([치료 후 체중-치료 전 체중]/치료 전 체중× 100) 계산하여, 그 값이 5% 이상인 경우를 의미 있는 체중 변화로 정의하였다. 이에 따라 체중 변화군과 체중 유지군의 두 그룹으로 분류하였다.
환자들의 나이는 40세 이하와 초과군으로 나누었으며, American Joint Committee on Cancer (AJCC) TNM (tumor, node, metastasis) stage를 기준으로 하여 종양의 크기는 2 cm 이하, 2-5 cm, 5 cm 초과로 각각 T1, T2, T3로 구분하였고, 액와 림프절 상태는 전이가 1-3개, 4-9개, 10개 이상으로 각각 N1, N2, N3로 구분하였으며, TMN stage는 II, III로 구분하였다.
에스트로겐 수용체와 프로게스테론 수용체는 양성과 음성으로 구분하였고 HER2/neu는 3+인 경우를 양성으로, 2+인 경우 형광동소 교집법(fluorescence in situ hybridization, FISH) 검사로 HER2/neu 유전자 증폭 유무를 확인하였다.
유방암의 분자적 아형은 에스트로겐 수용체 양성이면서(또는) 프로게스테론 수용체 양성, HER2/neu 음성인 경우를 luminal A로, 에스트로겐 수용체 양성이면서(또는) 프로게스테론 수용체 양성, HER2/neu 양성인 경우를 luminal B로, 에스트로겐 수용체 음성, 프로게스테론 수용체 음성, HER2/neu 음성인 경우를 triple negative로, 에스트로겐 수용체 음성, 프로게스테론 수용체 음성, HER2/neu 양성인 경우를 HER2 type으로 각각 정의하였다.
BMI는 체중(kg)을 신장(m)의 제곱으로 나눈 값으로 하였고 항암치료 시작 당시 신장과 체중을 계산하여 과체중 기준인 25 kg/m2로 그 이상과 미만으로 분류하였다. 환자의 임상적 병리적 특징은 chisquare test로 단변량 분석을 시행하였고, binary logistic으로 다변량 분석을 시행하였다. P-value는 0.05 이하를 통계학적으로 유의한 것으로 하였다.

결 과

환자들의 항암 치료 시작 시 평균 나이는 49.6세(범위, 29-71세), 종양의 평균 크기는 2.8 cm (범위, 0.7-10 cm)으로 AJCC TNM stage상으로 T1이 36명(40.4%), T2가 44명(49.4%), T3가 9명(10.2%)이었으며, N1이 53명(59.6%), N2가 18명(20.2%), N3가 12명(20.2%)으로 TMN stage II가 52명(58.4%), stage III가 37명(41.6%)이었다.
에스트로겐 수용체 양성은 60명(67.5%), 음성은 29명(32.5%)이었고, 프로게스테론 수용체 양성은 58명(65.2%), 음성은 31명(34.8%)이었고, HER2/neu 발현은 양성이 52명(58.5%), 음성이 37명(41.5%)이었다. 분자학적 아형에 따른 분류는 luminal A가 28명(31.4%), luminal B가 33명(37.1%), triple negative가 9명(10.0%), HER2 type이 19명(21.5%)이었다.
평균 BMI는 22.5 kg/m2 (범위, 17.9-38.9 kg/m2)로 나타났으며 과체중에 해당하는 25 이상은 36명(40.4%), 25 미만은 53명(59.6%)이었다. 체중 변화는 평균값이 3.1 (범위, -6.35~16.7)로 5% 이상의 체중 변화는 37명(41.6%)으로 이 중 5명(13.5%)은 체중 감소였고 32명(86.5%)은 체중 증가였다. 5% 미만의 체중변화는 52명(58.4%)으로 이 중 13명(25%)은 체중 감소, 5명(9.6%)은 변화가 없었고, 34명(65.4%)은 체중 증가였다. 평균 추적 관찰기간 45.1개월 동안 영상의학 검사상 재발 소견을 보인 환자는 17명(19.1%)이었으며, 재발이 없는 환자는 72명(80.9%)이었다.
단변량 분석에서 액와 림프절 전이 여부(P=0.003), TNM stage(P=0.001)가 재발과 통계학적 유의성이 있는 인자로 나타났으며, 다변량 분석에서도 유의성이 있었다. 환자의 나이, 에스트로겐 수용체, 프로게스테론 수용체, HER2/neu 발현, 종양의 크기, BMI는 통계학적 유의성이 없었으며, 체중 변화가 있는 그룹과 없는 그룹과의 재발 여부차이(P= 0.610) 또한 통계학적 유의성이 없는 것으로 나타났다(Table 1).

고 찰

본 연구에서 항암 치료 중 발생한 체중 변화와 재발과의 상관 관계를 통해 체중 변화가 유방암의 예후 인자로서 의미가 있는지를 알아보고자 했다. 최근 연구들에서 유방암을 진단받을 당시 과체중인 환자가 정상 체중인 환자에 비해 체중 변화가 더 많고, 더 나쁜 예후를 가진다는 연구 보고가 있으며[12], 항암치료 중 체중 증가나 체중 감소를 보였던 환자들은 체중 변화가 없었던 환자들에 비해 더 나쁜 예후를 가진다는 보고도 있다[13,14]. 또한, 과체중인 환자가 과도한 체중감소(> 10%) 시에도 더 나쁜 예후를 가진다는 보고도 있다[15].
본 연구에서는 항암 치료 후 유의한 체중 변화를 보인 군이 전체 89명 중 37명(41.6%)으로 나타났으며, 체중 변화와 유방암의 재발과 유의한 상관 관계나 나타나지 않았다. 서양의 다른 연구에서 유의한 체중 변화가 나타나는 것과 비교해 봤을 때[10,16], 국내연구에서는 유의한 체중 변화를 보이지 않기도 한다[17]. 본 연구에서는 중간 BMI 값이 22.5 kg/m2로 서양 연구[18,19]의 중간 BMI 값 24.1-27.2 kg/m2에 비해 상대적으로 낮으며, 과체중 환자가 적다는 점 등이 유의한 체중 변화에 영향을 줄 수 있고 이에 따라 유의한 체중 변화가 나타나지 않거나 분석에 영향을 줄 수 있다[20].
또한, 몇몇 이전의 연구 결과에 따르면, AC항암 요법을 시행한 유방암 환자에서 치료기간 동안 유의한 체중 증가가 없었고, 체중 증가와 예후의 상관 관계가 없다는 보고도 있어 유방암 환자에서 항암제 치료가 체중 변화에 미치는 영향과 예후에 대한 분석은 아직까지 불분명한 상태이다[19-22].
Fisher 등[23]에 발표에 따르면 AC 항암 요법에서 5% 이상의 체중 변화를 보인 환자군은 14.4%로 CMF (cyclophosphamide+methotrexate+5-FU) 항암 요법의 42.2%보다 낮은 수치를 보였고, 장기간의 다제 항암치료 요법을 받은 사람에서 체중 증가가 더 많이 나타나는 보고[18]도 있어 항암제 종류에 따른 체중 변화의 차이에 대한 연구도 필요하겠다.
유방암 환자에서 일부 예후인자와 BMI는 연관이 있는 것으로 보고되고 있으며[24], 많은 연구에서 BMI가 클수록 종양의 크기가 큰 것으로 보고하고 있다[25,26]. Daling 등[27]에 의하면 BMI가 큰 유방암 환자에서 높은 종양 등급, 높은 세포분열, 큰 종양 크기를 보이며, 많은 유사한 연구에서 액와 림프절 전이가 많음을 보고하고 있다[28]. 이러한 연구들에 의하면 비만한 환자에 있어서 지방 조직이 난소 이외의 에스트로겐 자극의 원인이 될 수 있다는 것과, 비만한 사람에서 에스트로겐 전구체가 증가되고 에스트로겐으로의 전환이 증가됨을 원인으로 들고 있다.
그러나 본 연구 결과는 BMI와 재발은 연관성이 없는 것으로 나타났다. 국내의 한 연구[29]에서도 유방암 환자의 예후와 관련한 BMI의 영향을 알아보기 위해 BMI 20 미만의 저체중군, 20 이상 25 미만의 정상 체중군, 25 이상의 과체중군으로 나누어 5년 예상 전체 생존율과 무병생존율을 조사하였으나 유의한 차이점을 찾지 못했다. 이처럼, BMI, 체중 변화와 관련한 유방암 환자의 예후에 대한 분석은 서양의 연구 결과와 국내 연구 결과가 상이한 경우가 많다.
본 연구는 후향적 연구이면서 비교적 케이스가 작았다는 제한점은 있었으나, 국내 환자를 대상으로 한 연구로 이전의 서양 환자를 대상으로 한 연구와의 차이점을 비교 분석할 수 있었다는 점에 의의를 가질 수 있겠다.
본 연구 결과에서는 체중 변화와 재발과의 상관 관계가 없는 것으로 나타나 항암 치료 중 발생한 체중 변화를 예후 인자로 보기는 어렵겠다. 그러나 다른 연구에서 상이한 여러 가지 결과들이 나오고 있으며 서양과 국내 환자 간 인종, 생활 습관, BMI 등 차이점이 많은 바 국내 환자를 대상으로 하여 지속적으로 더 많은 데이터 수집과 대규모 환자를 대상으로 한 임상 연구가 필요할 것으로 생각된다.
